With rapidly changing demographics over the past several years, the population covered by the CTDHP may no longer be what you thought. The CTDHP covers 850K people, which is 1-in-4 residents in the State of Connecticut. There are CTDHP members in every town in the State. In addition, the CTDHP can help you with flexible participation requirements, comprehensive support services, and customized patient referrals!
Join us! If you are ready to enroll you will need some of the following information to get started:
- Federal and State Tax ID
- Your NPI
- A copy of your liability insurance
- Your specialty certificate
- A copy of your diploma
- Office owners will need a W-9 and EFT information.
