Provider Survey Announcement

The 2015 Provider Survey is underway. A representative from The Connecticut Dental Health Partnership (CTDHP) will be calling provider offices to update our referral information. The survey will only take a few moments to complete and will allow CTDHP to refer clients to your office based on the information your office provides us. The survey information allows CTDHP to more precisely match a client's needs to a provider's service capability.
