Effective 9/1/2015 The fluoride code D1206 has been added to the dental fee schedule. Please disregard our guidance from 2013 regarding the billing of dental procedure codes D1206 and D1208. Please bill these codes based on the description in the CDT 2015 or the most current version in the future. The guidelines that describe the benefit for these procedures can be found in our most recently updated benefit grid found under the tab for Provider Reference Material.
